Sean the explanation for the above behavior (both equations are numbered "one") is that the rendering engine renders equations lazily and independantly. so there's not really any easy way of having equation numbering work on a per page basis. oh well.

What about the nested alignment environments. It might be AMSLaTeX?. Here is a working example. Notice also that LaTeX? comments seem to be correctly handled
